Denovo Hypertension

Case sheet:

Chief Complaints:
A 60yr old male, resident of Atmakur, labourer by occupation came to OPD with chief complaints of Diminition of vision in right eye since 1yr and blurring of vision since 1yr

History of presenting illness :
Patient was apparently asymptomatic one year back then he developed Diminition of vision in his right eye which was insidious in onset and gradually progressive in nature.  It was not associated with pain, discharge, watering and coloured halos. He then consulted a general physician during which he was diagnosed with hypertension 

Past history:
Patient is a known case of hypertension 
N/K/C/O DM,  Asthma, TB, CAD 

allergic history : Not significant

Drug history: not significant

Personal history:
Diet: mixed
Appetite: normal
Sleep: adequate 
Bowel and bladder movements : regular 
Addiction : occasional alcohilic,  smoking 1pack/day

Family history: Insignificant 

General examination :
No pallor
No icterus
No cyanosis
No clubbing of fingers
No lymphadenopathy
No pedal edema

Vitals:
Temperature : 98.6F
Pulse rate: 84bpm
Respiratory rate: 18cpm
BP: 140/100 mm Hg
SpO2: 98

Systemic examination :

Respiratory system :
Vesicular breath sounds heard
No additional breath sounds heard

Cardiovascular system :
S1 S2 heard
No murmurs

Abdomen:
Soft, non tender
Scaphoid

CNS:
No neurological deficits seen

Investigations

Provisional diagnosis : Immature senile cataract with De novo hypertension
